Description

ESSENTIAL FUNCTIONS/RESPONSIBILITIES:

• Assists with monitoring and coordinating the delivery of preventive and routine health care for individuals with developmental disabilities.
• Adheres to and assists with ensuring facility compliance with all Physician, NP, PA, etc. orders.
• Completes required nursing care documentation promptly.
• Assists with coordinating medical program needs with community services, medical providers and other related agencies.
• Assists with ensuring adequate scheduling and completion of health related exams, appointments, laboratory/diagnostic testing as ordered or required.
• Safely transports individuals to and from appointments if necessary.
• Assists with the monitoring of medication administration in the facility.
• Administers medications, treatments and supplements as assigned.
• Communicates issues and carries out all directives of overseeing RN and Nursing Administration.
• Participates as an active member of the Interdisciplinary Team.
• Maintains compliance with and assists with ensuring facility compliance with all applicable State/Federal regulations and agency policy and procedure.
• Maintains communication with Medical Liaison, Site Management Team, Service Coordinator and other staff as necessary.
• Participates in various nursing workgroups as assigned.
• Provides nursing coverage to all People Inc programs as directed by Nursing Administration.
• Performs CPR and Strategies for Crisis Intervention and Prevention (SCIP-R).
• Complies with all agency policies and procedures.
• Other duties as required by Nursing Administration.


MINIMUM QUALIFICATIONS:


• Current NYS licensure to practice as a Licensed Practical Nurse.
• NYS Drivers License that meets agency policy.
• Ability to meet physical requirements of the position when performing the following: Standing, walking, sitting, bending, stooping, squatting, kneeling and climbing.
• Lifting requirements of 35 lbs.
• Ability to lift and transfer individuals manually and mechanically as required by prescribed program/plan.
• Ability to move, reposition, and place individuals in pieces of equipment.
• Physical agility and ability to react to emergency situations, including maintaining Strategies for Crisis Intervention and Prevention (SCIP-R) certification as per regulatory guidelines.
• CPR certification required.
