Full Job Description
The City of Chesapeake is seeking a compassionate and organized Community Programs Specialist I to serve as the Thrives Case Manager. This position plays a key role in supporting the Chesapeake Thrives initiative by providing individualized case management services to residents and families. The ideal candidate will be community-focused, driven, and skilled at coordinating and delivering holistic care that is safe, timely, equitable, and client-centered.

Key Responsibilities:
- Provide direct case management for individuals and families referred through the Thrives Resident Referral Process
- Conduct outreach and engagement to connect residents with appropriate services and resources
- Assess residents' needs, develop tailored service plans, and coordinate service delivery
- Monitor progress and adjust service plans as needed to meet client goals
- Build and maintain relationships with community service providers
- Collaborate with internal and external partners to ensure effective service coordination
- Work independently under the general supervision of the Thrives Coordinator, planning and implementing work methods and project timelines
