Nurse Practitioner with Experience in Congestive Heart Failure (Full-Time)

We’re currently seeking a compassionate and skilled Nurse Practitioner to join our growing team full-time. In this role, you’ll focus on the specialized care and management of patients with Congestive Heart Failure (CHF), working in close collaboration with our cardiologists to deliver comprehensive and evidence-based treatment.

Key Responsibilities:

• Evaluate, diagnose, and manage patients with CHF
• Develop individualized treatment and medication plans
• Monitor patient progress and adjust care as needed
• Educate patients and their families on disease management and lifestyle modifications
• Collaborate with the broader care team to ensure continuity of care

Requirements:

• Minimum of 2 years of experience managing CHF patients
• Strong clinical assessment and diagnostic skills
• Excellent communication and patient education abilities
• A deep commitment to improving patient outcomes and quality of life
