Invitation to Bid
Crossroads Health Early Head Start/Head Start is seeking proposals from a registered Dietician/Nutrition Service Providers to provide nutrition services consultation to pregnant mothers and children ages birth to kindergarten enrolled in the Early Head Start (EHS) and Head Start (HS) program for the time period July 1, 2025-June 30, 2026. Dietician/Nutrition services will include case consultation and review, assessing nutritional status of children, snack menu review, offering resources to families, and in-service training of staff and parents. Proof of insurance coverage will be required and, if available, should be submitted with proposals.
Request for Proposal information will be available beginning at 9:00 a.m. on April 28, 2025 and can be picked up at Crossroads Health Early Childhood Services, 1083 Mentor Avenue, Painesville, Ohio 44077, or by calling (440) 358-7370. Responses to this RFP are due no later
than 11:00 a.m. on May 19, 2025 at 1083 Mentor Avenue, Painesville, Ohio 44077. Two (2) copies of the proposal must be submitted in a sealed envelope marked “Proposal Crossroads Health Early Head Start Supportive Services: Attention: Shannon Vocca. All proposals will be
opened at that time. The Crossroads Health Board of Directors reserves the right to accept or reject any or all bids, or any portion thereof, and to award a contract which the Board of Directors decides to be in the best interest of the program.
