JOB SUMMARY:
This full-time position is responsible for providing education, counseling, and Medical Nutrition therapy and care to patients. This is done via assessment, development, implementation, documentation, and monitoring of nutrition plans.
ESSENTIAL FUNCTIONS:
* Provides registered dietitian services according to policies and procedures, and federal and state requirements.
* Guide qualifying patients to our Food Pantry in accordance to policies and procedures.
* Plans, organizes, develops, and directs the nutritional care of pantry patients and referred patients in accordance with current federal, state and local standards, guidelines and regulations.
* Assesses /Monitors the patients' nutritional status and provides recommendations to clinical/ medical staff.
* Develop and update nutritional plans as needed.
* Educates patients, families, and staff on nutritional concepts and diet modification.
* Works with other members of the interdisciplinary team to ensure that modified texture diets and therapeutic diets follow the patient's medical condition.
* Reviews changes to ensure compliance with the policy and procedures and state and federal guidelines.
* Conduct audits of relevant nutritional care on a routine basis.
* Completes nutritional assessments on referred patients, quarterly, annually, and with any change in condition as per guidelines.
POSITION REQUIREMENTS:
Education/Experience
* Full-time Registered Dietitian or new grad/RD eligible
* Minimum of Bachelor's degree in Dietetics from an accredited college or university
* Registered by the Commission on Dietetic Registration of the Academy of Nutrition and Dietetics or eligible.

* An active Dietetics License in the state of Pennsylvania or eligible
* Registered Dietitian with Commission on Dietetic Registration, renewed every 5 years
* Licensed Dietitian Nutritionist (LDN) with the Commonwealth of Pennsylvania, renewed every 2 years
* Experience working in a Federally Qualified Health Center (FQHC) and/or similar community health settings.
Skills/Abilities
* Interest in community-based programming.
* Ability to appreciate and enjoy working with individuals.
* Sufficient understanding of windows-based computer applications to learn and use the electronic medical record and organize one's schedule and required reports.
* Knowledge of principles of counseling and education.
PHYSICAL REQUIREMENTS:
While performing the duties of this job, the employee is regularly required to sit; use hands to manipulate objects, tools or controls; reach with hands and arms; and talk and hear. The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and the ability to adjust focus. Noise level in the work environment is usually quiet.
