Scope
The Weekend Hospice Nurse offers skilled and compassionate care to patients on weekends and holidays. This role involves managing clinical needs, coordinating with interdisciplinary teams, and addressing urgent situations promptly. The Hospice Nurse ensures that patients and their families receive consistent, high-quality care in accordance with the hospice care plan. Serving as the primary clinical contact during the weekend off-hours, the Hospice Nurse is essential in providing continuity, comfort, and trust during a crucial phase of the care journey, possessing emotional resilience and a thorough understanding of hospice philosophy and practice.
Compensation: $1,250.00/weekend Friday 5 p.m. - Monday 8 a.m. (approx)
Mileage Reimbursement: 50 cents per mile
Expectations
- Serve as the case manager for patients during the weekend coverage period.
- Conduct skilled nursing visits in alignment with the IDG (Interdisciplinary Group) plan of care.
- Coordinate client care with social workers, chaplains, in-home aides, physicians, and volunteers.
- Assess patients and families promptly for changes in condition or urgent needs.
- Provide client and family education in end-of-life care, medications, and care techniques.
- Communicate effectively with physicians and the Hospice Administrator regarding patient status.
- Document accurately and in a timely manner, including medication use, DME, supplies, and care interventions.
- Participate in scheduled weekend calls, visits, and evaluation visits as needed.
- Respond to urgent needs including deaths, symptom crises, or equipment issues.
- Leave clear documentation and care plans in the home for continuity by weekday staff.
- Assist in the recertification and revocation processes as needed.
- Ensure personal and patient safety by adhering to infection control and safety protocols.
- May participate in orientation or act as a preceptor as assigned.
- Promote and represent the nursing profession and Healthkeeperz's mission positively in the community.
- Weekend Schedule: Weekend shift begins Friday at 5:00 PM and ends Monday at 8:00 AM, or following morning handoff/report.

Qualifications
- Active North Carolina RN license
- ADN required; BSN preferred
- Minimum one year of hospice or home health experience preferred
- Excellent assessment, triage, and communication skills
- Strong organizational skills with ability to work independently
- Comfortable using mobile devices and electronic documentation platforms
- Reliable transportation and willingness to travel to patient homes
Physical Demands
- This role requires prolonged periods of driving, visiting patient homes, and working on a computer.
- Frequent bending, reaching, lifting, talking, and listening are necessary.
- The nurse must be able to lift and move equipment and supplies, and manage physical tasks in patient care settings.
