We're looking for a compassionate and experienced Hospice Nurse to join our team in Essex and Union County NJ. In this role, you will provide comprehensive care and support to patients and families during some of life's most vulnerable moments. As a Hospice Nurse Case Manager, you'll coordinate all aspects of patient care, working closely with our interdisciplinary team to ensure each individual receives dignified, comfort-focused care.

• To be considered for the role of Hospice Nurse Case Manager, a valid RN license issued by the state of New Jersey is mandatory.
• The candidate must possess current certifications in BLS (Basic Life Support) and ACLS (Advanced Cardiac Life Support).
• While previous experience in hospice care is preferred, it is not a strict requirement for this position.
• Essential qualifications include excellent communication skills, strong interpersonal abilities, and a proven track record of effective teamwork.
