We are currently seeking a Clinical Social Worker IIto join our GRAT team in Seattle, Washington.

What will YOU do?

The Clinical Social Worker II responds to client concerns, provides comprehensive assessments and develops interventions that increase client stability, support independence, and prevent hospitalization. The Clinical Social Worker II is a WA State Department of Health Licensed Independent Clinical Social Worker (LICSW), a Licensed Mental Health Counselor (LMHC), or has an equivalent behavioral health license.

Pay Rate:$90,358.80 per year

Work Schedule:Monday to Friday,40 hours per week

  • Responsibilities:

  • Initiate home and/or virtual visits with client.
  • Engage clients to identify needs through in-depth assessment utilizing evidence-based tools, considering cognitive, behavioral, social and functional aspects.
  • Investigate complex psycho-social situations with collateral contacts, environmental observations, and when possible, review of prior attempts to engage client.
  • Develop and implement strengths-based care plan with client.
  • Mobilize clients' families and support circles to respond to client needs.
  • Facilitate appropriate referrals to community programs, health care providers, APS and/or law enforcement. Advocate for needed services and provide necessary follow-up.
  • Seek and integrate cultural guidance from diverse community members/agencies about how to best serve individual clients.
  • Perform crisis intervention; as a Mandatory Reporter as defined by state law, immediately report identified concerns such as abuse, neglect, emergent medical issues, etc. to appropriate state or local authority and notify supervisor; comply with other related laws, organization policies, and program procedures.
  • Advance care plans, track progress, and close contact when interventions are substantively accomplished.
  • Participate in team meetings to review assessments and interventions in case conference, to further strategize, collaborate, and confer with multidisciplinary team.
  • Assist Supervisor with program planning and operations. Provide observations related to programmatic development to Supervisor.
  • Participate in mandatory program and organizational activities, meetings and trainings.
  • Requirements:

  • WA State Department of Health independent clinical social work licensure (LICSW), mental health counseling licensure (LMHC), or other equivalent behavioral health licensure.
  • Experience performing psychosocial, psychiatric, and substance use assessments, and developing interventions relevant to risk for the adult population.
  • Minimum three (3) years of professional experience working with adults with cognitive or behavioral health issues.
  • The ability to travel to various areas of King County using a mode of reliable transportation to meet clients and community groups during scheduled worktime.
