Bachelor Level - Intensive Care Coordinator     Are you passionate about making a positive impact on the lives of children and families?      We have an exciting opportunity for a Bachelor Level Intensive Care Coordinator to join our growing children's mental health program at CSA in Somerville. We take pride in serving a culturally diverse population and value the unique perspectives, backgrounds, and experiences of the families we support. We offer excellent supervision towards licensure as well!     As a Bachelor Level Intensive Care Coordinator, your responsibilities will include:  
  • Provide comprehensive care coordination to children and families.
  • Perform initial clinical assessments to identify family and youth's need and goals.
  • Develop, implement and monitor individualized care plans based on family needs and goals.
  • Facilitate care plan team meetings.
  • Act as a liaison with collateral agencies and programs
  • Advocate for families and educate them on self-advocacy
  • Identify and assist youth and families in accessing necessary services
  • Ensure proper documentation, and safety planning
  • Participate in staff meetings, supervision, and training
Schedule: This position offers a hybrid schedule with flexible options. On-call availability during evenings and weekends on a rotating schedule is required to ensure continuous support for our clients.  

Pay Rate: 

  • $22.28/hour
    • additional $0.96/hourly compensation for language skills

Required Skills

  • Excellent communication and organizational skills 
  • Basic computer fluency, including Microsoft Office
  • Valid driver's license and reliable, personal vehicle

Required Experience

  • Bachelor's degree in a mental health-related or human services field (such as social work, counseling, child development, psychology, human services) from an accredited college or university
  • Minimum of one-year experience working with children with serious emotional disturbance
  • Community-based experience strongly preferred
