Ascend Medical is seeking a compassionate and experienced Nurse Practitioner or Physician Assistant to provide relationship-centered care at a premier Assisted Living facility in Decatur, GA. This 16-hour per week role is ideal for a provider who is passionate about geriatrics and values quality time with patients over quantity.

What You'll Do
- Provide compassionate, relationship-based care to older adults in an assisted living environment.
- Build trust with patients, families, and facility staff through clear communication and consistent presence.
- Manage chronic conditions, address acute concerns, and tailor care plans to the unique goals of each patient.
- Collaborate with the facility's care team to ensure seamless, coordinated support.
- Apply your home health or hospice experience to improve outcomes for medically complex patients.
- Maintain flexible availability for patient visits and on-site presence.

Qualifications
- Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in Georgia (unrestricted, non-probationary).
- Master's degree from an accredited program.
- Minimum of 3 years of clinical experience, with strong preference for geriatric, hospice, or home health background.
- Active DEA license (or eligibility to obtain).
- Availability to work on-site in Decatur; occasional visits to Metro Atlanta locations may be required.
